Foster/Adoption Dependent
- Subscriber must complete a Foster/Adoption Dependent Form.
- Completion of a Foster/Adoption Dependent form must be received within 60 days of the following events to be effective on the requested date:
- Child(ren) adopted or pending adoption (effective as of the date of birth or as of the date legal proceedings were started.
- Foster child (Welfare Department assigned under State law).
Child for whom the subscriber is legal guardian or has legal custody (court stamped documentation will be required).
COBRA
The Consolidated Omnibus Reconciliation Act (COBRA) of 1986 provides a continuation of employer group insurance benefits at the group rate for employees and their dependents after certain qualifying events occur for specified lengths of time.
- When a participant loses coverage, they must be offered Cobra (continuing insurance if they so desire to purchase it from your district).
- The terminated employee would be eligible for 18 months at 100% charge payable to your district
- If the dependent who loses coverage is the divorced spouse or an over-age child, (s)he would be eligible for 36 months at 100% charge to district.
College Students

GAHP covers dependents to December 31st of the year they turn 19.
- If the child is a full time college student, he/she would continue to be covered. GAHP requires proof of full time college status. The census form and documentation from the college must be provided. If the child is not a full time college student, he/she will be dropped effective 12/31 of the current year.
- In August/September of each year the GAHP office will send a college census form to districts to be distributed to each subscriber with dependents in that age group. The information is updated annually.
Handicapped Dependent Eligibility
- Unmarried children of any age who are incapable of self-support because of a handicap and are fully dependent on the parent(s) are covered.
- The handicap condition must have existed while the child was a dependent on the parent’s policy (prior to the dependent/student age limitation).
- A Handicapped Dependent application must be completed and signed by both the subscriber and the medical doctor. This application must be received prior to the dependent age limitation and will be reviewed by Excellus BC/BS to determine eligibility for handicapped status.
Medicare
- When retiree turns 65, Medicare becomes primary.
- If subscriber is retired and is NOT yet 65 but spouse turns 65, Medicare is primary for spouse and GAHP primary for subscriber.
- If subscriber turns 65 and becomes Medicare primary, and the spouse is not yet 65, the spouse remains GAHP primary.
- Contact the school district Business Office to complete the proper forms.
