The purpose of this form is to verify your request, your identity as the parent/legal guardian, as well as your understanding that this information is confidential. Upon completion of the form, please follow the directions that will appear in the enclosed cover letter along with your request form. You will then be scheduled, at a mutually convenient time, to review the requested composite score.
