Genesee Valley LPN Documentation Checklist
Please follow the steps below to submit your application for the GV BOCES LPN Program. If you have any questions, please call (585) 344-7788.
Please Note: Due to some organizations' processing requirements, some documentation may take time for you to obtain. It is strongly advised to start requesting your documentation at the start of the application process.
Step 1 Application and TEAS testing documentation:
_____ Submit your application
_____ Submit a $75.00 non-refundable processing fee over the phone, by credit card, to our Business Office at 585-344-7704, or send a money order (no personal checks) payable to Genesee Valley BOCES. Send to Genesee Valley BOCES LPN Program, 8250 State Street Road, Batavia, New York 14020.
Once we receive both your application and the $75.00 non-refundable processing fee. The candidate will be notified to schedule a time to take the TEAS test. TEAS Study and testing information.
Step 2 Application Processing Documentation
After successful passing of the TEAS test, we require the following documentation, before we can proceed with processing your application:
_____ Submit a 1,000-word minimum, double-spaced, 12-font, typed essay stating "Why I want to become an LPN." Complete and submit your essay using this link.
_____ Include three letters of reference (one from a supervisor and two from co-workers). Have your references completed and submit letters of reference using this link.
_____ Submit High School transcript/GED
If the applicant scores high on matrix qualifications, the candidate will be notified and an interview will be scheduled.
Step 3 Program Acceptance Documentation
After the interview, if the candidate is selected for the LPN Program, the candidate will be notified and will require the following documentation:
Medical Documentation
_____ CDC Covid-19 vaccination and booster card
_____ CPR for health care professional BLS (Adult, AED, and Child Care)
_____ Health physical within last 6 months
_____ Proof of 2 MMR immunizations or positive titers for each (M, M, R)
_____ Proof of TD immunization (within the last 10 years)
_____ Proof of PPD test within the last 6 months
_____ Proof of flu vaccine within the last 8 months
_____ Proof of Varicella immunization or positive titer

Prerequisites for Admission
The candidate must be at least 17-years-old and provide proof of a High School Diploma or GED.
Successfully complete the steps and requirements of the admission process, as guided on this website.
To participate in the classroom, lab, and clinical activities, students must be able to perform the following activities with NO restrictions:
- Sit, stand, walk, bend, and reach for long periods of time in accordance with the length of the clinical day.
- Lift > 35 lbs., push, pull, carry, grasp, twist, hold/support extremities, or persons (e.g. during transferring, ambulating, turning, moving, or lifting patients in beds).
- Manipulate equipment that requires both fine and gross motor skills such as, but not limited to wheelchairs, beds, IV pumps, blood pressure cuffs, and computer keyboards.
- Maintain full functional use of all senses, including feeling, hearing, seeing, speaking, and detecting odors for activities such as, but not limited to responding to alarm sounds, performing patient assessments, and reading medical records.
- Sit, stand, walk, bend, and reach for long periods of time in accordance with the length of the clinical day.

Transcripts
A high school diploma or high school equivalency diploma/transcript is required to apply to the GV BOCES Practical Nursing program.
If you have graduated from high school, contact the high school you attended and request to have your OFFICIAL transcript sent directly to us at:

**NOTE: If you have to request a duplicate from NYSED, the wait time to receive it is approximately 8-10 weeks. Please act quickly if you need a duplicate. **Please note: We must receive transcripts from any/all previous nursing school attempts. If you have previously attended the GV BOCES Practical Nursing Program, please request that your transcript be copied to your application folder.

Step 2: Process and Documentation

After you pass both the READING and MATH TEAS tests. The following documentation is required before we can process your application:
High School Transcript /GED
Include documentation of an original "official" transcript with a seal from a United States High School sent directly from the school to us in a sealed envelope or an original "official" GED transcript (with grades and diploma attached) sent directly to us in a sealed envelope from Albany (will cost you $10.00). Essay
Submit a 1,000-word minimum, double-spaced, 12-font, typed essay stating "Why I want to become an LPN." Complete and submit your essay at the following using this link.
References
Include three letters of reference (one from a supervisor and two from co-workers). Have your references complete and submit letters of reference using this link. There is also a link to a PDF version at the bottom of this page.
Submitted documentation and TEAS scores will be reviewed by admissions staff for interview consideration.
Upon consideration, staff will notify the candidate to set up an interview date and time.
The application process for acceptance/denial is based on a point system and being accepted into the program can be very competitive. Refer to page 11 in the LPN Handbook.
After all the documentation and an interview have been completed, the admission staff will make their selections. Candidates will be notified whether or not they were accepted/denied into the program.
The Genesee Valley BOCES LPN Nursing Program admits students whose ability, preparation, and character indicate potential success in the program. The selection of students is based on a review of credentials, references, pre-entrance test scores (TEAS), and health status, without regard to race, sex, age, national origin, religion, marital status, or political affiliation.

Refund Policy
Students who withdraw or who are terminated prior to the end of any enrollment period will be eligible for a tuition refund (Term 1 less tuition deposit and book/supplies allowance) for the term they withdraw according to the following schedule:

Financial Aid Appointments
All applicants are required to attend an Individual Financial Planning Session to design a payment plan to cover their program’s direct cost of attendance. Financial Aid information and application assistance are provided by the Financial Aid Office to help students and their families fully understand their financial obligations prior to enrollment.
After you have met with a Financial Aid Advisor and have successfully completed your financial aid application, your eligibility for Federal Pell Grants and Federal Direct Loans will be estimated. You will be provided with an Individual Financial Plan that will summarize your expenses and Financial Aid during your enrollment in the Genesee Valley BOCES. If your Financial Aid is not sufficient to cover your entire costs, the Financial Aid Advisor will discuss additional financing options to assist you and your family in meeting your expenses.
